WHAT IS DIABETES?

 

Diabetes is a serious disease. It means your blood glucose (often called blood sugar) is too high. Your blood always has some glucose in it because your body needs it for energy. But too much glucose in your blood is not good for your health.

 

 

WHY SHOULD I CONTROL MY DIABETES?

 

If you control your diabetes, it can help you feel better and stay healthy. Keeping your blood glucose close to normal lowers your chances of having heart, eye, kidney, and nerve problems. To control your diabetes, you need to know your blood glucose numbers.

 

 

TYPE I

 

Type I diabetes is usually diagnosed in children and young adults, and was previously known as juvenile diabetes or insulin-dependent diabetes mellitus (IDDM). In type I diabetes, the body does not produce insulin. Type I diabetes may account for 5% to 10% of all diagnosed cases of diabetes. This form of diabetes usually strikes children and young adults, although disease onset can occur at any age.

 

 

TYPE II

 

Type II is the most common form of diabetes. In type II diabetes, either the body does not produce enough insulin or the cells ignore the insulin, and were previously known as non-insulin-dependent diabetes mellitus (NIDDM) or adult-onset diabetes. Insulin is necessary for the body to be able to use sugar. Type II diabetes may account for about 90% to 95% of all diagnosed cases of diabetes. It usually begins as insulin resistance, a disorder in which the cells do not use insulin properly.

 

Finding out you have diabetes is scary. But don't panic. Diabetes is serious, but people with diabetes can live long, healthy, happy lives.

 

 

WHAT DOES IT COST TO ENROLL IN HOW DO I QUALIFY TO ENROLL?

 

There is no cost to enroll in the plan.  

 

There are no monthly plan premiums after a member enrolls. In fact, the Member has $0 out of pocket cost for Member benefits, which may even include diabetic supplies. There are no hidden fees or costs.

 

The plan is available to individuals at any age, in every state and every county who, by doctors orders, are required to check their blood glucose level and have Medicare or Medicaid.  Individuals who are enrolled in a Medicare Advantage Plan or have private insurance are also eligible to enroll.

CARE COORDINATOR

 

Our Care Coordinator Services are available to you as part of being a Member. A Care Coordinator will contact each Member to access their current diabetic needs.

 

Our Care Coordinators help with a wide range of diabetic issues that may affect a Members overall well-being.  Our Care Coordinators work very closely with each Member to monitor and identify any changes and offer solutions that address the Members total diabetic needs.  Diabetes unchecked can progressively worsen and can be life threatening.  Our Care Coordinators assist Members with diabetes control and management.

 

 

24-HOUR NURSE HOTLINE

 

Our 24-hour Hotline Nurses are an immediate, reliable and caring source of health information and available to you.  This 24-Hour Nurse Hotline service is available to Members who otherwise could not receive the answers to non-emergency health related questions or inquiries when the Members primary care physician is unavailable or cannot be reached.
